LPA continuation sheets

The form has limited space. Continuation sheets carry the overflow — extra attorneys, extra wording, or a donor who cannot sign.
What each sheet is for
| Sheet | Use it when |
|---|---|
| Continuation sheet 1 | You have more attorneys, replacements or people to notify than the boxes allow |
| Continuation sheet 2 | Your preferences or instructions do not fit in section 7 |
| Continuation sheet 3 | The donor cannot sign and someone signs at their direction, with two witnesses |
| Continuation sheet 4 | Additional information the OPG has asked for or a second certificate provider signs |
Rules that catch people out
- Every continuation sheet must be signed and dated in the same sequence as the main form.
- Sheets must be submitted with the LPA — they cannot follow later.
- If the donor signs at direction (sheet 3) you need two witnesses, not one.
- Do not staple over signature boxes or reorder pages.
A missing sheet is a rejection
If section 7 refers to a continuation sheet that is not enclosed, the OPG cannot register the LPA and returns the whole application.
Avoiding them altogether
Two attorneys, one replacement and no instructions fits on the standard form with no continuation sheets at all — which is one more reason to keep preferences and instructions short. Where a sheet is genuinely needed, our pack prints it, fills it and lists it on your checklist.
Common questions
Is a continuation sheet a sign of a complicated LPA?
Not necessarily — a large family often needs sheet 1 simply for space. Sheet 2 is the one worth pausing over.
Can I write on the back of the form instead?
No. Anything not on the official sheets risks rejection.
